Your First Appointment
Because first impressions go a long way in establishing future relationships, we make every effort to ensure your first appointment exceeds your expectations.
First appointments are a great opportunity to become familiar with our dental team and office environment. You can expect to be greeted by a friendly staff member who will gather necessary dental and health information, complete forms and learn about your current concerns and/or problems with your smile. Financial and insurance information will be discussed along with a brief overview of our practice. Our staff will also be happy to answer your questions and plan appointment times that best fit your schedule.
Please provide at least 24 hour notice for any cancellations, or a missed-appointment fee of $25 may be assessed. This courtesy notice helps us to make the most of our time together.
After you meet us in person, we know you will want to stay!

What to Expect
A first appointment typically includes a professional dental cleaning and examination by a dental hygienist at which time x-rays may be recommended. One of our dentists will evaluate your exam and personally meet with you. Working together, a personalized treatment plan will be developed that best supports your dental care and budget.
Dental Insurance
We are happy to accept all traditional forms of dental insurance. Here are just a few of the insurance plans for which we are participating in-network providers: Aetna, Anthem, Delta Dental, Guardian, Humana, MetLife, Principal Financial Group and Cigna.
We know dental insurance can be confusing. Our front office is extremely knowledgeable about dental insurance, and how to utilize it best for your situation. We are committed to helping you understand your treatment options and how your dental insurance benefits will apply.
Please bring your dental insurance information with you to your first dental appointment.
NOTE: We currently do not accept Medicaid, Molina or CareSource.

Before visiting us, please take the time to print out and complete the following online new patient, medical history and HIPPA compliance forms. This valuable information is important to your care and will be saved in your permanent records. Bringing these completed forms to your first appointment saves valuable time and allows us to focus on your dental needs.
